In a pioneering advance in the field of male infertility, the Indian Council of Medical Research (ICMR) has unveiled an Artificial Intelligence-based tool in collaboration with Amity University. This innovative mechanism, designed to detect microdeletions in the Y chromosome—a genetic anomaly contributing significantly to male infertility—holds the potential to transform how in-vitro fertilisation (IVF) outcomes are anticipated for affected individuals. During a recent discussion, Dr. Ananya Mehta, a renowned geneticist, provided deeper insights into the creation and implications of this AI tool. Dr. Mehta underscored the often-overlooked reality that male factors contribute to nearly half of all infertility cases. Among these, Y chromosome microdeletion (YCMD) emerges as a pivotal genetic cause, impeding sperm production. Men afflicted with YCMD, unfortunately, find their sperm count unresponsive to conventional medical treatment, highlighting the pressing need for alternative solutions such as this AI tool.

Fittingly named the Fertility Predictor, this tool serves as a beacon of empowerment for many couples. “Our objective transcended merely developing another diagnostic device,” explained Dr. Mehta. “We aimed to equip couples with the knowledge to make informed decisions about their reproductive journey.” The path to creating this tool was marked by meticulous data collection over two years, involving more than 500 men with YCMD undergoing Assisted Reproductive Technology (ART). Researchers applied sophisticated machine learning algorithms to this rich dataset, enabling the AI to forecast outcomes like sperm retrieval rates, fertilisation success, and even live birth probabilities.

The Fertility Predictor boasts an impressive accuracy rate of approximately 80 per cent, offering a degree of predictability that was previously unattainable. “This isn’t just a question of hope,” Dr. Mehta stated. “It’s about providing couples with a data-driven choice.” A notable feature of the tool is its ability to deliver specific numerical probabilities for clinical pregnancy and live birth chances, tailored to the individual’s precise YCMD type. Dr. Mehta highlighted the significance of this precision, noting that it offers couples a personalised insight into their fertility prospects.

Nonetheless, the tool is accompanied by a significant ethical consideration. Dr. Deepak Modi, a leading scientist at ICMR, emphasised that male offspring conceived through IVF from fathers with YCMD will inherit this genetic defect, which will result in infertility in subsequent generations. “Prospective parents must weigh this aspect carefully,” Dr. Mehta advised, acknowledging that while the tool facilitates the path to parenthood, it also requires careful contemplation of genetic transmission over the long term.
